چکیده

Prompt malaria diagnosis is crucial so antimalarial drugs and supportive care can then be rapidly initiated. A 15-year-old boy who had traveled to Africa (South Africa, Kenya, and Nigeria between January 3 and 25, 2011) presented with fever persisting over 5 days, headache, diarrhea, and dysuria, approximately 17 days after his return from the journey. Urinalysis showed pyuria and hematuria. Blood examination showed hemolytic anemia, thrombocytopenia, disseminated intravascular coagulation, and hyperbilirubinemia. Plasmapheresis and hemodialysis were performed for 19 hospital days. Falciparum malaria was then confirmed by peripheral blood smear, and antimalarial medications were initiated. The patient's condition and laboratory results were quickly normalized. We report a case of severe acute renal failure associated with delayed diagnosis of falciparum malaria, and primary use of supportive treatment rather than antimalarial medicine. The present case suggests that early diagnosis and treatment is important because untreated tropical malaria can be associated with severe acute renal failure and fatality. Physicians must be alert for correct diagnosis and proper management of imported tropical malaria when patients have travel history of endemic areas.

Acute kidney injury in a shepherd with severe malaria: a case report

Malaria is one of the main reasons for outpatient consultation and hospitalization in Mauritania. Although four Plasmodium species, ie, Plasmodium (P.) falciparum, P. vivax, P. malariae, and P. ovale, cause malaria in Mauritania, recent data on their frequency is lacking. Since infections with P. falciparum generally result in serious disease, their identification is important. We report a case...

Pathophysiology, clinical presentation, and treatment of coma and acute kidney injury complicating falciparum malaria

PURPOSE OF REVIEW Cerebral impairment and acute kidney injury (AKI) are independent predictors of mortality in both adults and children with severe falciparum malaria. In this review, we present recent advances in understanding the pathophysiology, clinical features, and management of these complications of severe malaria, and discuss future areas of research. RECENT FINDINGS Cerebral malaria...

Rapid Diagnosis of acute kidney injury (AKI) associated with cardiac surgery, using the liver type fatty acid binding protein (L-FABP) biomarker

Abstract Background and objectives: cardiac surgery is often associated with acute kidney injury (AKI). Nowadays, AKI is typically diagnosed by an increase in serum creatinine, which is a delayed and unreliable biomarker. Recent studies recommended using the liver type fatty acid binding protein (L-FABP) as an early biomarker. Material and Methods: The urine samples of 18 adult patients undergo...

Diffuse alveolar hemorrhage due to Plasmodium falciparum: a rare entity--are steroids indicated?

Severe falciparum malaria is common in tropics, patients usually present with any of the following manifestations or combination of cerebral manifestations, circulatory failure, liver failure, acute lung injury and acute kidney injury. Pulmonary manifestations in falciparum malaria usually include acute respiratory distress syndrome (ARDS) and acute lung injury (ALI)1. Here, we present two case...

Urinary bile casts in bile cast nephropathy secondary to severe falciparum malaria

BACKGROUND Severe cholestatic jaundice may complicate with bile cast nephropathy (BCN) causing severe acute kidney injury (AKI). In this study, we investigate BCN in severe falciparum malaria complicated with jaundice and AKI. METHODS This prospective study was conducted in a tertiary health care institution with high prevalence of malaria. A cohort of 110 patients with falciparum malaria com...
